500 mg PO q12h

Indication-Specific Adjustment

Exclusions to usual dosing:

CNS infections: 500 mg q6h - 8h

Clostridioides difficile-associated colitis: 500 mg q8h (only if oral vancomycin unavailable)

Parasitic/amoebic infections: discuss with ID consult service for recommendations

H. pylori: frequency depends on regimen utilized, please see H. pylori CustomID page

May consider more frequent dosing (q8h) for complicated infections without source control such as undrained large abscesses or infections involving prosthetic material (e.g. aortic grafts, aortoenteric fistulae, etc)

 

Other indications:

Bacterial vaginosis: 500 mg q12h

Pelvic Inflammatory Disease (in combination with other antibiotics): 500 mg q12h

Trichomoniasis: 500 mg q12h

 

Doses other than above will result in an automatic switch per P&T protocol

Fixed (ie non weight-based) doses in adults are historically based on a 70 kg patient. Specific disease states or individual patients may warrant dosages that differ from the above recommendations. Since product-specific criteria for dose adjustment based on creatinine clearance exist, consult product information regarding specific recommendations for dosage adjustment based on estimated creatinine clearance.
