Criteria
Daytime (7am to 5pm): ID TELEPHONE APPROVAL
Nighttime (5pm to 7am): ADULT Meropenem orders do not require ID pre-authorization, however these antimicrobials are reviewed by a member of ASET and appropriate follow up for ID approval will be discussed when necessary.
Per Protocol: Exception noted in the Adult Febrile Neutropenia protocol for patients presenting with severe sepsis (attached)
Restricted Use
Meropenem (vs imipenem-cilastatin) is the preferred broad-spectrum anti-pseudomonal carbapenem available at Duke University Hospital.
