Drug-Specific Information
Except CNS toxo and PCP with sufa allergy (600mg q6h), other CNS infections (600g q6h), and necrotizing fasciitis (900mg q8h), doses of 600mg q6h, 900mg q6h and 900mg q8h will result in automatic switch to 600mg q8h per P&T-approved protocol
IV to PO Conversion:
600 mg IV q8h = 450 mg PO q8h OR 300 mg PO q6h
