Bronchitis, CAP, uncomplicated skin AND/OR subcutaneous tissue, maxillary sinusitis, pharyngitis, tonsillitis, otitis media 300mg q12h 300mg once daily 300mg x1, followed by 300 mg 3 times per week postdialysis on dialysis days, with an additional 300 mg dose 48 hours into each 72-hour interdialytic period*

*If dosed MWF on dialysis days, administer an additional 300 mg dose on Sunday
