Bronchitis, CAP, uncomplicated skin AND/OR subcutaneous tissue, maxillary sinusitis, pharyngitis, tonsillitis, otitis media	300mg q12h	300mg once daily	300 mg q48h and 300 mg at the end of each hemodialysis session (with subsequent doses given every other day)
