2.67g ONCE followed by 1g IV q6h 1.8g ONCE followed by 900mg IV q8h 1.33g ONCE followed by 900mg IV q12h

*each dose infused over 3 hours

^for iHD dosing: administer dose after hemodialysis on dialysis days.

Drug-Specific Information

Aztreonam/avibactam susceptibility testing is not routinely performed.

Add metronidazole if sufficient anaerobic coverage is required.
